School Report & Transcript Request

This form is to be completed by the authorized school official (e.g., principal, and counselor) at the school the candidate presently attends. It should be returned to us directly from the school along with an official, up-to-date, complete (Grades K-5) transcript, including all standardized test scores. School Report and transcripts should reach us no later than January 11, 2008.
